• (Rural Health Unit) Sanitary/ Health Card

• (BFP) Fire Safety Inspection Certificate – FSIC

• (BFP) License to Operate (For LPG/ Butane dealers)

• (MARKET) Lease Contract (for LGU owned stalls/ spaces)

• (MAYOR’S OFFICE) Endorsement (For poultry / piggery)

• (TOURISM) – Certificate of Compliance for Accommodation establishments (Hotels, resorts, lodging house, apartelle )

• Establishments such as banks, restaurants, malls, tourist destinations, and beaches for day use for statistics data gathering.
